MAI-CoC Data Resources

 MAI-Continuum of Care (MAI-CoC) Program Data -Resources:
These resources are updated as an ongoing reference for the MAI-CoC program, specific to new RHHT forms, and ongoing TRAC NOMS GPRA MAI-CoC and TRAC IPP MAI-CoC.

This update and documentation pertains to grantees under the MAI-CoC braided program, RFA/FOA TI-14-013, but does not represent the data requirement in its entirety.

The data tools are authorized and intended for use only as directed under the MAI-CoC grant program. MAI-CoC grantees must be guided by their on-site Project Director, Government Project Officer (GPO), or other appropriate guidance, i.e., RFA, Notice of Award, as well as updated information on program or data requirements. Rapid HIV and Hepatitis Testing (RHHT):

For questions pertaining to RHHT form and reporting, please contact the RHHT Form Helpdesk at the following email: Project_HIVconsolidatedEval@abtassoc.com or by phone: 844-282-5881.

In June, 2016, the Project Director and/or designated RHHT staff were provided access information to the online RHHT reporting system, replacing the ‘paper data collection’ for RHHT. Grantees are required to enter RHHT data directly into the web based data system, for the Rapid Testing Data Collection System at https://rhht.samhsa.gov

RHHT form (PDF read-only format) – SAMHSA Rapid Testing HIV and Hepatitis Testing Clinical Information Form (OMB No. 0930-0343 Expiration Date: 12/31/2018) - Update April 2016
This document is a copy of the OMB approved SAMHSA Rapid Testing HIV and Hepatitis Testing Clinical Information Form. All grantees are required to offer HIV and hepatitis testing to all of their clients and family members or significant others who may be at risk for HIV or viral hepatitis. Note: The client ID number used in this RHHT form for a particular client receiving behavioral health services should be the same client ID number that is used for the same individual in the TRAC NOMS data collection and reporting system. Please continue to use the secure mailbox for RHHT form submission.

This resource is aimed to provide supplemental information for how providers should address each question on the RHHT form. 


TRAC NOMS, Program Specific Questions and IPP for MAI-CoC Program

For questions pertaining to TRAC IPP and TRAC NOMS, please contact the TRAC Help Desk at TRACHELP@westat.com or 1-855-796-5777

TRAC NOMS for Client Level Services Data and Program Specific Questions:
The MAI-CoC program requires the completion of TRAC NOMS for all clients who receive treatment for mental and substance use disorders treatment. Please note, any generic language in TRAC specific to ‘mental health’ may be read as being specific to both mental and substance use disorders treatment, as appropriate. Also note, the client ID number used in TRAC NOMS for a specific client should be the same client ID number which is used for the same individual on the RHHT form. (See the added FAQ TIP below.)

FAQ TIP: Is a TRAC NOMS interview required 6 months after client’s discharge? The TRAC NOMS does not include an interview 6 months after discharge, but it does require a 6-month reassessment during the period the client is receiving services.

The form may be downloaded for use in hard copy, and then entered into the online system.

This QXQ guide is for TRAC NOMS client level services. This resource is aimed to provide supplemental information for how providers should address each question in the TRAC-NOMS tool. See posting >

TRAC NOMS MAI-CoC Program Specific Questions: – Held separately, not entered into TRAC NOMS online. (No changes, but NOTE the supportive guidance in the NEXT item, below.) This data is gathered from the client medical record, for clients with HIV positive status. This is required under Section H of the TRAC Adult Client Services Tool. This item cannot be ‘entered’ as part of TRAC NOMS but is held separately, securely on-site, until further notice. See posting >

FAQ TIP: Do grantees need to enter or update IPP from Year One (Sept 2014-Sept 2015)? Yes, any added or updated Year One IPP activities (from Sept 2014-Sept 2015) for the MAI-CoC should be reported in the first quarter of Year Two; if reported last year, do not reenter. If adding Year One IPP data, add the words, “in grant year one…” in the indicator description, and then submit.
