Summer Cohort

2018 INNOVATION COMMUNITY TOPICS


BUILDING INTEGRATION IN PEDIATRIC CARE SETTINGS

This Innovation Community's purpose is to create greater capacity to deliver efficient and effective behavioral health services in the pediatric primary care setting. Participating organizations will include rural, suburban, and urban pediatric settings including federally qualified health centers, private pediatric practices, and school-based health centers. The focus of the improvement activities will be to develop the capacity to integrate services. Participants will either be new to integrating behavioral health services or just beginning their work (e.g., hiring a BH consultant and/or introducing a mental health or substance use screen to their services array, etc.). Participants will receive information and support using a variety of learning methodologies including but not limited to individual and group coaching calls to address the educational needs, provided resources, and tools, group emails, and webinars.

IMPLEMENTING PAIN MANAGEMENT GUIDELINES IN INTEGRATED CARE SETTINGS

This Innovation Community will support integrated behavioral health and primary care providers in understanding the policies and practices that support and help implement evidence-based guidelines for pain management. Participants will take part in individual and small group coaching calls, webinars, and group email discussions that will address their educational needs related to pain management and provide practical resources and tools for the creation or further development of evidence-based pain management guidelines. By the end of this Innovation Community, participants will have identified goals, and have created a work plan that describes in detail the actions and activities each organization will carry out to achieve their goals.

IMPLEMENTING TEAM BASED CARE

This Innovation Community will support primary and behavioral health providers to understand and adopt team based approaches to care. Research describing how to efficiently and effectively deploy and support an integrated team of primary and behavioral health providers will be reviewed. The National Committee on Quality Improvement (NCQA) team-based care standards and an organizational self-assessment will be used to help teams build their work plan. Participants will have the opportunity to engage experts from organizations that have experience delivering team base care. All teams will report-out on IC work plan progress during the final webinar and learn through individual coaching calls, reading and video viewing assignments, and group email conversations.

This Innovation Community will support behavioral health and primary care providers in understanding the policy and trends shaping value-based payment methodologies, the payment reform continuum, and the transformations required in clinical and business practices to succeed under value-based contracts. Participants will take part in individual and small group coaching calls, webinars, and list serve discussions that will address the educational needs of participants and provide practical resources and tools. By the end of this Innovation Community, participants will have completed a readiness assessment, identified concrete goals, and created a work plan that lays out their next steps and tools needed to achieve their stated outcomes.
