  • The Agency for Healthcare Research and Quality's webpage, Questions are the Answers, provides information and guidance for patients and clients on asking questions and providing information to doctors and other care providers.
     
  • The Patient-Centered Primary Care Collaborative offers a video and brochures (in both English and Spanish) to help patients and families understand what a medical home is and the patient-centered philosophy behind it.
     
  • A Family Guide: Integrating Mental Health and Pediatric Primary Care, produced by the National Alliance on Mental Illness, helps family members understand how to get involved in the integrated care movement and improve the care their children receive in pediatric primary care settings.
     
  • Caregivers Count Too! is a toolkit designed to help practitioners assess the needs of family caregivers. The toolkit suggests that successful care, whether from hospitals, home, or community-based settings, depends upon knowing the needs of both the care recipient and the family caregiver.
     
  • The Circle of Support Approach Self-Assessment (COSA-OSA) enables an organization to assess the degree to which their activities align with key domains that characterize a highly supportive, engaging approach to treatment. This tool provides an organization with a comprehensive set of activities that align with the engagement of social supports in a manner that supports the client’s goals and needs.
